We are excited to announce that we have recently welcomed a new population health nurse to our clinical team. At JMHS, we prioritize the health and wellbeing of our community as a whole. By maintaining good health, we can prevent long-term illnesses and reduce the cost of healthcare, which is especially crucial in today’s economy. Many people may be curious about what exactly a population health nurse does. Essentially, their role involves working with different populations or age groups to ensure that they receive the appropriate care and services. Four key areas that fall under the umbrella of population health include chronic care management, quality and safety, public health, and health policy. At JMHS, we are constantly seeking opportunities to enhance the overall health of our community. If our population health nurse reaches out to you, please keep in mind that our goal is to provide you and our community with the best possible care and support.
