Our providers and staff are working collaboratively with the Minnesota Department of Health and following guidance from the Centers for Disease Control & Prevention. On Friday afternoon, November 12, the Centers for Medicare & Medicaid Services (CMS) issued a revision of visitation guidance for Long-Term Care Facilities. CMS notes that the reason for visitation restrictions during the COVID-19 public health emergency were to mitigate the opportunity for visitors to introduce COVID-19 into the nursing home.
Residents have the right to make choices about aspects of his or her life that are significant to the resident. Choosing to have visitors is one of those rights. Please review the following summary of changes to LTC visitation:
- Residents/tenants are allowed to have any “indoor” visitor he/she chooses, regardless of vaccination status or age, subject to very limited exceptions.
- There is no limit on the frequency/length of visits.
- There is no limit on the number of visitors a resident can have at one time; however, visits will be conducted in a manner that adheres to the core principles of COVID-19 infection prevention.
- To accommodate safe physical distancing, four visitors will be allowed in the resident’s room.
- Conference Room C will be used for gatherings of 5-10 visitors, and for any visitors under the age of 12 years old whenever possible. These accommodations will promote the core principles of infection prevention.
- When the Lac qui Parle County COVID-19 community transmission rate is substantial or high, all residents and visitors should wear face coverings and physically distance, regardless of vaccination status.
- Currently, Lac qui Parle County and almost all counties in MN are in high community transmission status.
- Visitation is allowed during an Outbreak Investigation; Visitors will be made aware of the potential risk of visiting during an outbreak investigation and adhere to the core principles of infection prevention.
- COVID-19 Antigen Card testing will be offered to visitors during substantial or high levels of community transmission.
- Outdoor and window visits continue to be encouraged, weather permitting.
Resident/Tenant Outings
Fully Vaccinated* residents and tenants are allowed to leave the building and attend outings with family and friends. Outings must be scheduled in advance and resident/tenants must sign in and out of the campus.
- Fully Vaccinated Residents should continue to:
- Comply with any current state mandates or restrictions on group size at the time of the outing
- Fully vaccinated residents/tenants do not need to quarantine upon their return of their non-medically necessary outing.
- Non vaccinated residents/tenants may be required to quarantine following a non-medically necessary outing.
Visiting rights may be restricted or revoked for individuals if communicated safety measures are not followed.

On Tuesday, December 13, 2022, two staff members tested positive for COVID-19. These individuals had direct contact with both staff and residents. In accordance with CMS and CDC guidelines, we are currently in outbreak status and have initiated contact tracing outbreak testing protocols.
Guidance states that asymptomatic health care professionals with a higher-risk exposure and residents with close contact with someone with COVID-19 infection, regardless of vaccination status, should have a series of three viral tests. Testing is recommended immediately and if negative, again 48 hours after the second negative test, and if negative again 48 hours after the second negative test. This will typically be at day 1(where day of exposure is day 0), day 3, and day 5. As always, you will be contacted if your loved one was exposed and needs to be tested.

The resident has been moved to our COVID-19 room in the Care Center. This room was updated to have the necessary air exchange filtration system to care for our positive residents. In addition, it houses a separate space for staff to safely put on and take off the required PPE. This space was created so that we could safely and properly care for residents that test positive for COVID-19.

JMHS Closes Care Center COVID Unit Most of the residents will come off of quarantine this week and the River Road neighborhood will once again become a neighborhood and not a COVID-19 Unit. Residents will be able to join in small group activities, and eat in the dining rooms – six feet apart of course. The assisted living tenants will also be able to resume normal non-outbreak activities next week.
December 21, 2020 the hospital and clinic front line staff will receive the first dose of the COVID-19 vaccine and vaccinations in the care center and assisted living will begin at the end of December. The COVID-19 vaccine provides hope for a new chapter in the fight against this virus. The vaccine works with the immune system to help fight the virus if you are exposed. Getting these shots will help protect our residents, tenants, staff and community from being infected; and will help us take a big step towards ending this pandemic.

As of December 2, 2020 Johnson Memorial Health Services (JMHS) has identified 11 positive COVID-19 Care Center Residents. To protect all our residents’ health and safety, JMHS has created a COVID-19 Unit located on the East Hall of the River Road Neighborhood by creating a barrier wall on the west side of River Road’s kitchen/dining area. The unit currently has space for 14 residents but can be expanded if necessary. All positive COVID-19 residents have relocated to the COVID Unit, where they are being monitored and quarantined appropriately.
In addition, JMHS has hired specific contract staff to work in the COVID-19 unit to minimize staff from moving between the COVID-19 unit and the non-COVID-19 unit in the Care Center. All persons entering the COVID-19 must be in full droplet precautions, utilizing N-95 masks, eye protection, gloves, and disposable gowns.
As a result of the outbreak, yesterday, we tested all residents with either a rapid IDNOW test or a State-issued Antigen-Based Point-of-Care (POC) test for COVID-19 in LTC facilities. The State-issued Antigen-Based POC tests take approximately 15 minutes for the result. They are not as sensitive as the IDNOW or PCR tests but did identify two additional positive COVID-19 residents yesterday. Starting December 2, each LTC staff member must take a rapid Antigen-Based POC COVID-19 test before beginning work.

Last week’s round of testing was first conducted on November 23, with results received on November 27, show 100% negative COVID-19 test results for the long-term care staff, tenants, and residents. Testing again occurred on Friday, November 27, and we are waiting on results.
Residents and tenants will be tested on Monday, November 30, 2020 and weekly until there are 0 positive cases for 14 days. LTC Staff will be tested on Mondays and Fridays until we fall below the 10% CMS County Positivity Rate for LqP County with no positive staff, resident, tenant cases in the past 14 days. Lac qui Parle is currently at 22.4% positivity rate and is surrounded by counties well over 10%. The most recently posted CMS average positivity rate in Minnesota is 17.4%. We continue to do all we can to protect the health and safety of our resident, tenants and staff.

Last week’s round of testing was first conducted on November 16, with results received on November 19, show 100% negative COVID-19 test results for the Care Center staff, tenants, and residents. Testing again occurred on Friday, November 20, and we are waiting on results. We have, however, had Care Center staff test positive this past weekend. The staff is quarantined. The residents that these staff cared for are on precautions, and will be tested with our normal testing parameters as long as they remain asymptomatic. All symptomatic residents, tenants, and staff are tested immediately and put on precautions.
The resident/tenant family contact will be contacted if your loved one is put on precautions Residents and tenants will be tested on Monday, November 23, 2020 and weekly until there are 0 positive cases for 14 days. The October 7 Assisted Living results showed 100% negative COVID-19 positive results for both tenants and staff.

Johnson Memorial Assisted Living: Although the CMS COVID-19 Testing for Nursing Homes requirements do not apply to Assisted Livings, JMHS has been using the same COVID-19 testing requirements for the Johnson Memorial Assisted Living. The September 21, 2020 second round of COVID-19 testing on Assisted Living Tenants and Staff, with results on September 24 show 100% negative COVID-19 test results for the Tenants. Unfortunately, one Assisted Living Staff tested positive. This employee is currently on quarantine. Consequently, the Assisted Living Tenants and Staff will continue to be tested weekly until they receive 0 positive cases for 14 days. JMHS also follows the CMS Nursing Home guidelines regarding screening and visitation.

Our providers and staff are working collaboratively with the Minnesota Department of Health and following guidance from the Centers for Disease Control & Prevention. We are taking all appropriate measures and doing everything possible to minimize the spread of the virus in our facility and protect the health and safety of all who live and work here. Efforts to mitigate transmission include:
- Enhanced infection control precautions
- Screening residents, staff, and preapproved, compassionate care visitors for symptoms
- Restricting visitation and entry of people to the building
- Testing staff twice a week and residents weekly at a minimum, up to daily when needed in outbreak circumstances, as testing supplies allow. All CC workers are also tested before each shift, as testing supplies allow.
- Postponing or carefully limiting communal activities, including social distancing and other infection control precautions

As of December 2, 2020 Johnson Memorial Health Services (JMHS) has identified 11 positive COVID-19 Care Center Residents. To protect all our residents’ health and safety, JMHS has created a COVID-19 Unit located on the East Hall of the River Road Neighborhood by creating a barrier wall on the west side of River Road’s kitchen/dining area. The unit currently has space for 14 residents but can be expanded if necessary. All positive COVID-19 residents have relocated to the COVID Unit, where they are being monitored and quarantined appropriately.
In addition, JMHS has hired specific contract staff to work in the COVID-19 unit to minimize staff from moving between the COVID-19 unit and the non-COVID-19 unit in the Care Center. All persons entering the COVID-19 must be in full droplet precautions, utilizing N-95 masks, eye protection, gloves, and disposable gowns.
As a result of the outbreak, yesterday, we tested all residents with either a rapid IDNOW test or a State-issued Antigen-Based Point-of-Care (POC) test for COVID-19 in LTC facilities. The State-issued Antigen-Based POC tests take approximately 15 minutes for the result. They are not as sensitive as the IDNOW or PCR tests but did identify two additional positive COVID-19 residents yesterday. Starting December 2, each LTC staff member must take a rapid Antigen-Based POC COVID-19 test before beginning work.
Efforts to Mitigate Transmission at JMHS:
- All LTC Staff, Tenants, and Residents will be tested for COVID-19 on Monday, December 7, and weekly going forward until 0 positive cases for 14 days. Testing frequency is also contingent on the Lac qui Parle County Positivity Rate per CMS.
- All Positive Residents will be moved to the COVID Unit to be treated and cared for by separate contract staff.
- Staff will continue to wear surgical masks and face protection in all patient areas as source control.
- Staff will continue to be “Actively Screened” prior to starting their shifts, including temperature scans, symptom checks and completing a rapid Antigen-Based POC COVID-19 test.
- Residents/Tenants will continue to be “Actively Screened” each day, including temperature scans and symptom checks.
- Small group activities will be conducted as appropriate and with safety precautions.
- The beautician and barber will continue to be “Actively Screened” and will work out of the Salon.
- Zoom meetings with family and friends will be encouraged and supported.
- In-person visits are suspended during “Outbreak Status”. We know this is so difficult, and will do our very best to resume in-person visits as soon as possible.

On November 29, 2020, Johnson Memorial Health Services (JMHS) has identified a positive COVID-19 Care Center Resident. The Care Center resident is being monitored and has been quarantined accordingly. In addition, their family has been contacted.
Last week’s round of testing was first conducted on November 23, with results received on November 27, show 100% negative COVID-19 test results for the long-term care staff, tenants, and residents. Testing again occurred on Friday, November 27, and we are waiting on results.
Residents and tenants will be tested on Monday, November 30, 2020 and weekly until there are 0 positive cases for 14 days. LTC Staff will be tested on Mondays and Fridays until we fall below the 10% CMS County Positivity Rate for LqP County with no positive staff, resident, tenant cases in the past 14 days. Lac qui Parle is currently at 22.4% positivity rate and is surrounded by counties well over 10%. The most recently posted CMS average positivity rate in Minnesota is 17.4%. We continue to do all we can to protect the health and safety of our resident, tenants and staff.

Johnson Memorial Health Services (JMHS) recently completed COVID-19 testing on Care Center staff, tenants, and residents in accordance with guidelines provided by the Centers for Disease Control (CDC), Centers for Medicare & Medicaid Services (CMS), and Minnesota Department of Health (MDH). Last week’s round of testing was first conducted on November 16, with results received on November 19, show 100% negative COVID-19 test results for the Care Center staff, tenants, and residents. Testing again occurred on Friday, November 20, and we are waiting on results. We have, however, had Care Center staff test positive this past weekend. The staff is quarantined. The residents that these staff cared for are on precautions, and will be tested with our normal testing parameters as long as they remain asymptomatic. All symptomatic residents, tenants, and staff are tested immediately and put on precautions.
Residents and tenants will be tested on Monday, November 23, 2020 and weekly until there are 0 positive cases for 14 days. LTC Staff will be tested on Mondays and Fridays until we fall below the 10% CMS County Positivity Rate for LqP County with no positive staff, resident, tenant cases in the past 14 days. Lac qui Parle is currently at 17.1% positivity rate and is surrounded by counties well over 10%. The most recently posted CMS average positivity rate in Minnesota is 16.7%. Although the CMS COVID-19 Testing for Nursing Homes requirements do not apply to Assisted Livings, JMHS has been using the same COVID-19 testing requirements for the Johnson Memorial Assisted Living. The September 21, 2020 second round of COVID-19 testing on Assisted Living Tenants and Staff, with results on September 24 show 100% negative COVID-19 test results for the Tenants. Unfortunately, one Assisted Living Staff tested positive. This employee is currently on quarantine.
