- $240 per 4-person team: Includes 18 hole green fees and lunch.
- $40 per cart rental
- $5 per Mulligan

- Registration: 9:30 am
- Shotgun Start: 10:00 am
- Lunch: 11:30 am – 1:00 pm

- 1st Place: $400
- 2nd Place: $300
- 3rd Place: $200
- Plus: Hole and Door Prizes

All proceeds from the Golf Fore! Health Tournament are going towards the Assisted Living refresh.
